2 Qs for panelists...
1. Panelists observations regarding higher rates of occurrence of mucormycosis in patients who remain admitted in ICU than in patients of non ICU, at home & qmake shift hospitals, despite being immune compromised.
2. Secondly during this covid period, we learnt that our doctors and scientists are much more efficient, still why we compare our observations with international data. Although in foreign countries too Indian scientsts are putting their best
Thanks

I slightly disagree. Exenteration is a bit drastic for this case.
Also one should be careful about mixed Aspergillus and Mucor infections. I would like to give Liposomal Amphotericin.
In my personal series I have done retrobulbar clearance of necrotic tissue with good results. Vision wont improve obviously but its worthwhile to leave the orbit

I have a theory about this being similar to skull base osteomyelitis - like in Malignant Otitis Externa , response is titrated with ESR / CRP . Sometimes it may just be impossible to say if the osteomyelitis was bacterial / fungal, so best to treat both even if an overkill

Before this sudden spike of Mucormycosis , I have seen similar picture with Necrotizing Gingivostomatitis where it was an Anerobic infection which would respond to appropriate cover. But in this setting we have seen Mucor which shows up later if not sooner.
